Reminder to all 5^th grade students who will be entering 6^th grade in September 2012

All students born on or after January 1, 1997 and entering or attending grade SIX or an ungraded special education program will be required to have the additional immunizations given no earlier than the 10^th birthday.

DTP/DtaP (diphtheria, tetanus, pertussis).  Unless there has been less than 5 years since the last Td (tetanus, diphtheria) immunization.

AND

One dose of Meningococcal Vaccine

Children who are transferring into a New Jersey school from another state or country after September 1, 2008, shall receive one dose of Tdap, provided at least five years have elapsed from the last documented Td dose.  They also shall receive one dose of meningococcal vaccine.
