Registration

Kindergarten school assignments will be determined in June and parents will be notified at that time. School assignment is based on special education needs and enrollment numbers. It is not based on location. Please see Board Policy 5120 - Assignment of Students for further information. 

A child must be 5 years old on or before October 1st of the school year. See Board Policy 5112 - Entrance Age.

Kindergarten orientation will take place at your child's assigned school on a date to be determined.

REGISTRATION MUST BE COMPLETED ONLINE.

Required supporting documents for registration are:

• Original Birth Certificate
• Three proofs of residency - see Residency Requirements & Forms for more information
• Most recent physical and proof of immunizations
• Kindergarten Packet - includes the Home Language Survey, Parent Questionnaire, Preschool Questionnaire Release (signature only) and Oral Health Assessment
• IEP or 504, if applicable
• Custody papers, if applicable

When completing the online registration, continue clicking "Next Step" until all information is entered, then  click "submit" to send the information to the district. 

An automated email will be sent with a link to upload the required documents directly to the district. If your child has a physical in the summer, please submit the most recent physical and immunizations by June 1st and send the updated physical and immunizations after the summer appointment. Submit all documents as soon as possible.  If you are not able to upload the documents, please email them directly to colleen.smith@pequannock.org or drop them off at the Board of Education office located at 538 Newark Pompton Turnpike, Pompton Plains.

Please be aware that any initial determination of the student's eligibility to attend school in this district is subject to more thorough review and subsequent re-evaluation, and that tuition may be assessed in the event that an initially admitted student is later found ineligible.  If your student is found ineligible, now or later, you will be provided the reasons for our decision and instructions on how to appeal.

Elementary

Elementary school assignments are determined based on Board Policy 5120 - Assignment of Students and Board Regulation 5120R - Attendance Area Plan. School assignments will not be made until all registration documents are received.

Middle & High School

Grades 6-12 - Once all documents are received by the district, you will be contacted by a school counselor to make an appointment to review the transcript and schedule to determine a start date. This process may take a few days so it is best to begin the registration process as soon as you know your child will be attending the middle or high school.

Three residency documents are required for all new registrations and any change of address for current students. 

Proof of Residency
Must provide three first class documents with name, address, and date on each. Acceptable documents are as follows:

• Deed/lease
• Drivers license (or change of address confirmation)
• Property tax bill
• Utility bill
• Mortgage statement
• Financial account statement
• Voter registration
• Legal document with name, address, and date

Current Students

New Registrations
Upload residency documents through the link in the automated email you received after submitting the online registration.  

Where does your child live?  If any of the situations below apply to your family, please complete the corresponding affidavits and have them notarized.  

• Affidavit 1 - Student and parent/guardian live in a leased home where there is no written lease. Residency Affidavit 1 must be completed by landlord.
   
• Affidavit 2 - Student lives in Pequannock Township with someone other than parent. Residency Affidavit 2a must be completed by parent/guardian and Residency Affidavit 2b must be completed by Pequannock Township resident.
   
• Affidavit 3 - Student and parent/guardian temporarily live with Pequannock Township resident. Residency Affidavit 3a must be completed by parent/guardian and Residency Affidavit 3b must be completed by Pequannock Township resident providing temporary housing. Pequannock resident must supply three proofs of residency.

All affidavits must be original notarized documents. If needed, notaries are available at the Board of Education office. Residency documents are required for the resident providing housing.
