Preschool Special Services Registration

 

Welcome to Preschool Special Services Registration

Our preschool integrated program and the preschool disabilities program are both housed at North Boulevard School.  Preschool students are referred to the Special Services department which will then set up a meeting to discuss concerns.

 

 

Please contact Deirdre Tahan, secretary of Student Services, for information and instructions on how to refer your preschool child for special education services.  She can be reached at 973-616-6010 ext 3007 or deirdre.tahan@pequannock.org.

Once your child has been  referred for special education services please follow the instructions below.  

  • All registrations will be completed online using the pre-registration link   
  • When completing the online Pre-registration, continue clicking the "Next" button until all information is entered and questions answered and then click the Submit button to send the information to the district. 
  • An automated email will be sent with a link to upload the required documents directly to the district. Submit all documents as soon as possible.  If you are not able to upload the documents, please email them directly to Colleen Smith or drop them off at the Board of Education offices.
  • Required Supporting Documents for registration are:
    • Original Birth Certificate - must be sent prior to initial meeting
    • Three proofs of residency (see Residency tab for more information) - must be sent prior to initial meeting
    • Most recent physical and proof of immunizations
    • Preschool Special Education Packet  - includes the home language survey, parent questionnaire, and Oral Health Assessment
    • Evaluations/Early Intervention paperwork - please email to deirdre.tahan@pequannock.org
    • Custody papers, if applicable

Please have all registration documentation to the Board of Education office as soon as possible.  If your child has a physical in the summer, please submit the most recent physical and immunizations by August 1st and send the updated physical and immunizations after the summer appointment.

Please be aware that any initial determination of the student's eligibility to attend school in this district is subject to more thorough review and subsequent re-evaluation, and that tuition may be assessed in the event that an initially admitted student is later found ineligible.  If your student is found ineligible, now or later, you will be provided the reasons for our decision and instructions on how to appeal.
